Passing State Written and Skills Exams:

Skills Exam

  • Minimum score of 70% is required to pass the skills exam
  • Results of skills test and certificate are given at the conclusion of the test

Written Exam

  • Minimum score of 70% is required to pass the written exam
  • Scores are not mailed to student. The Registry does not receive numeric scores, only “pass” or “fail.”
How to Access Your Registry Status (After 2-3 week waiting period)

  1. Go to the following website: www.dia-hfd.state.ia.us
  2. Along the left side of your screen, you’ll see a box with navigational tabs; click on the“login” tab. This will bring up a large box in the middle of your screen with two smaller boxes requesting an acct ID and password. The acct ID is the last four digits of your social security number, an underscore and the first initial of your first and last names, in capital letters (example: 6789_AB). Your temporary password will be the first five digits of your social security number, an underscore and the first initial of your first and last names, in capital letters (example: 12345_AB).
  3. You will be prompted to set up five “baseline” or security questions.
  4. You will then be asked to create a new password that only you will know.
  5. This will take you to your own homepage within our database.  You can navigate anywhere within your record and you can print your card from this page as well.

  1. Call toll free 866/876-1997 to access the Voice Response System.
  2. Please follow instructions given to access information.
  3. A response that you are ACTIVE on the Direct Care Worker Registry indicates the successful completion of both the written and skills examinations.  Individual scores are not available from the registry.
